Given the expression one might assume everything - but actually this is not the case.
In almost every area of the world all inclusive car hire insurance means that there are some parts of the car that are still excluded. In Europe this tends to mean wheels, roof, underside of the car, and glass (normally both the windscreen and mirrors are excluded), whilst in the US typical limitations include:

Key replacement if you loose the key
Roadside assistance if you lock yourself out of the car
Damage to underside of vehicle if you are found to be driving 'off-road' which is not permitted
Damage to the roof of the car if you place goods on the roof or drive under a low barrier etc.
Damage to the interior (caused by spilling drinks, cigarette burns in seats etc)
